Excolo Christmas Party Giveaway Official Rules

NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.

1. Sponsor

The “Excolo Christmas Party Giveaway” (“Sweepstakes”) is sponsored by Excolo Medicine, located at 145 Del Orleans Suite C, Denham Springs, LA 70726. Phone: (225) 243-3666.

2. Eligibility

This Sweepstakes is open only to legal residents of the United States who are 18 years of age or older at the time of entry. Employees of Excolo Medicine and their immediate family members or individuals residing in the same household are not eligible to participate. Void where prohibited by law.

3. Sweepstakes Period

The Sweepstakes begins on December 1, 2024, at 12:00 AM (Central Time) and ends on December 14, 2024, at 8:00 PM (Central Time). Mail-in entries must be received by December 13, 2024, at 3:00 PM Central Time to be eligible.

4. How to Enter

Participants may enter the Sweepstakes through the following methods:

• Purchase Raffle Tickets: Each raffle ticket purchased provides one (1) entry.

• Purchase Services at the Christmas Party: Purchasing a service at the Excolo Christmas Party on December 14, 2024, provides two (2) entries per service.

• Purchase Packages at the Christmas Party: Purchasing a package at the Excolo Christmas Party on December 14, 2024, provides four (4) entries per package.

• Mail-In Entry: Send a 3x5 notecard with your full name, phone number, email address, and mailing address to Excolo Medicine, 145 Del Orleans Suite C, Denham Springs, LA 70726. Only one (1) entry per mail-in submission is allowed. Mail-in entries must arrive no later than December 13, 2024, at 3:00 PM Central Time to be eligible.

There is no limit to the number of entries through purchases. However, only one (1) entry is permitted per mail-in submission.

5. Prize

The Sweepstakes grand prize will be awarded to one (1) winner. The approximate retail value and description of the grand prize will be disclosed at the time of the drawing.

6. Winner Selection and Notification

The winner will be selected through a random drawing on December 14, 2024. The drawing will be conducted by Excolo Medicine. The winner will be notified in person at the event or via phone call. The winner must accept the prize within 24 hours of notification or the prize will be forfeited, and an alternate winner may be selected.

7. General Conditions

By participating, entrants agree to abide by these Official Rules and the decisions of Excolo Medicine, which are final and binding in all matters related to this Sweepstakes. Excolo Medicine reserves the right to cancel, suspend, or modify the Sweepstakes if any fraud, technical failures, or other factors impair the integrity or proper functioning of the Sweepstakes.

8. No Liability

By entering, participants release and hold harmless Excolo Medicine, its employees, and affiliates from any liability, claim, or damage arising from participation in the Sweepstakes or acceptance or use of any prize.
